The Self-Enablement Service (SES) is a community step-down service that is available in Hull and the East Riding for individuals with complex care needs, who require support to maintain their independence living in the community.

Who is this service for?

Our service is designed for individuals who need support to maintain accommodation and live independently. This includes:

  • Individuals with complex care needs.
  • Those accepted by the local authority as homeless.
  • People being discharged from long-term residential or hospital stays.
  • Individuals who have committed an offence.
  • Those leaving care.
  • Anyone else with identified complex needs that affect their ability to live independently in the community.

Who can benefit from our service?

Our Self-Enablement Service (SES) is available for individuals aged 18 and over who present with complex care and support needs. It is commissioned by Adult Social Care and Young People’s Teams and is delivered across Hull and the East Riding of Yorkshire.

The service is specifically designed for individuals who may be difficult to engage and/or are well known to local services due to the nature of their complex needs and challenges or severe disadvantage. This includes individuals who are being discharged from long-term residential or hospital stays, those who have a history of offences, individuals who are accepted by the local authority as homeless, individuals leaving care, and anyone else who has an identified need that affects their ability to maintain accommodation and live independently.

What does our Self-Enablement Service (SES) offer?

The Self-Enablement Service is a community step-down service that provides suitable self-contained accommodation where a program of outcomes-based care and support can be delivered. The service works in conjunction with statutory services involved with risk management and social care solutions to:

  • Provide social care and support for individuals with complex needs to sustain a safe and supportive home environment.
  • Reduce the risk and dependency of people entering long term care or acute settings.
  • Support rapid assessment through a hospital discharge process.
  • Develop people's capacity to live independently.
  • Develop people's ability to understand and identify daily coping strategies, including accessing wider support services.


What are the benefits of the service?

Benefits of the service include:

  • Reduced numbers of individuals entering care or residential settings inappropriately.
  • Reduced numbers of individuals having to access out of county placements due to the offer of a local community based solution.
  • Reduced numbers of individuals on a delayed discharge due to the offer of a local community based solution.
  • Provision of quality Personal Care that is regulated by the Care Quality Commission
