Full Job Outline for the position(s) of...

Social Inclusion Officer - 30hrs

Contract:

30 hours per week, Permanent (hours may vary across a rolling rota to include some evenings and weekends.)

Key Responsibilities:

Working under the direction of the organisation and service management teams and to adhere to contract requirements
Planning and providing person centred, strength based activities and groups to foster the wellbeing and social development of service users
Providing advice, guidance and support on access to local opportunities, groups and activities
Assessing and monitoring community needs and promote community cohesion to promote changes within the service and the wider community whilst developing systems for change and continual improvement
Recording and reviewing progress to ensure service users can fulfil their potential for independence, choice, inclusion and control

Essential Requirements:

Knowledge of the issues faced by customers that have been homeless, have complex needs or that have gone through the Criminal Justice System
Working experience of dealing with complex service users/customers/public
Working knowledge of the principles of risk assessment of an individual, environment or activity

Desirable Requirements:

Educated to NVQ Level 3 or equivalent
