This service assists the Local Authority in discharging its statutory duty to provide a range of support services to young people (aged 16-21) leaving the Looked After system, as laid out in The Children Leaving Care Act 2000.

The provision of 5 furnished units of accommodation with floating support is available to Young People who are clients of the 16+ Service in North East Lincolnshire. Referrals come via 16+ Service and after assessments and a joint decision process; suitable young people are placed, and supported, in their own accommodation.

Young people will be supported by Tenancy Support Staff and support will be structured around the preparation for a sustainable independent lifestyle. Accommodation is dispersed across the North East Lincolnshire area and young people are placed in an area which best suits their needs.

With tailored and effective packages of support this service ensures that the often difficult transition from being “looked after” to independence is a seamless and sustainable process.
