

A project that operates in partnership with the Safer Communities and the North Lincolnshire Primary Care Trust, and Humbercare. Humbercare provides tenancy support to 140 clients in their own properties throughout North Lincolnshire and is funded by the local supporting people team. Humbercare provides a substance misuse link officer, who works alongside tenancy support staff to provide advice and guidance in relation to alcohol and substances misuse issues. All of the support offered is provided on an outreach basis.
The project is designed to offer relapse prevention for clients who are experiencing problems maintaining tenancy. The staff can provide onward referrals for clients, who may need additional support concerning specialist treatment services. Support is offered and delivered in an agreed package, maximising client involvement and participation. Monthly support reviews take place with the client and when ready a short aftercare package of support can be arranged.
Referrals are accepted from: Statutory agencies, housing, community sectors and prospective clients. Humbercare provides a person centred service supporting clients to maintain an independent lifestyle within their own home. An out of hours service is provided in the support package and a newly introduced texting service is now operational.
