The Substance Misuse and Resettlement Team (S.M.A.R.T)is a project that operates in parnership with the Drug Action Team and Humbercare. Humbercare provides tenancy support to 15 clients in their own properties. Humbercare also provides a substance misuse link offficer, who works alongside tenancy support staff to provide advice and guidance in relation to alcohol and substances misuse issues. All of the support offered is provided on an outreach basis.

The project is designed to offer relapse prevention for clients who are experiencing problems maintaining tenancy. The staff can provide onward referrals for clients, who may need additional support concerning specialist treatment services. Support is offered and delivered in an agreed package, maximising client involvement and participation. Monthly support reviews take place with the client and when ready a short aftercare package of support can be arranged.
