The Substance Misuse and Resettlement Team (S.M.A.R.T)is a partnership between the Drug Action Team (D.A.T. Citysafe)and Humbercare.

The service aims to offer support to those with substance misuse issues within their own home and work with them to enable independent living free from the problems associated with alcohol and drug misuse.

The project has both accommodation support staff and a substance misuse link worker who work together to give a comprehensive package of support.

The support package includes advocacy, advice, information, assessment and brief interventions.

The service is for those who have the capacity to manage in their own tenancy with a planned package of support.
