The Self-Enablement Service is aimed to respond to people aged 16 plus that present with complex care and support needs.
The service is a community step down service for those customers who are difficult to engage historically and/or are well known to services locally due to the nature of their complex needs and challenges, they may need support to step down from acute or secure settings, residential settings, prison or hospital stays or due to their complex needs may need an intensive care and support package to maintain independence in their existing home within the community.
The service is designed to meet the provision of local person's who require care, accommodation and support.
The service provides or identifies suitable self-contained accommodation where a programme of outcomes based support can be delivered.
