Self Enablement Service

The Self-Enablement Service is aimed to respond to people aged 16 plus that present with complex care and support needs.

The service is a community step down service for those customers who are difficult to engage historically and/or are well known to services locally due to the nature of their complex needs and challenges, they may need support to step down from acute or secure settings, residential settings, prison or hospital stays or due to their complex needs may need an intensive care and support package to maintain independence in their existing home within the community.

The service is designed to meet the provision of local person's who require care, accommodation and support.

The service provides or identifies suitable self-contained accommodation where a programme of outcomes based support can be delivered.

The service works in conjunction with statutory services involved with risk management and social care solutions to:

  • Provide social care and support for a range of people with complex needs to sustain a safe and supportive home environment
  • Reduces the risk and dependency of people at risk of entering long term care or acute settings
  • Support rapid assessment through hospital discharge process
  • Develop people's capacity to live independently following the completion of time limited outcomes programme of care and support
  • Develop people's ability to understand and identify daily coping strategies, including accessing wider support services
  • Employs highly competent and skilled staff and management commensurate to the numbers of people dependant on the risk to staff, person and public
  • Is fully committed to proactive partnership working due to the close relationship that will need to exist between Adult Services, NHS, Probation and Community and Housing services
  • Is fully committed to working with a range of housing providers and landlords to provide a range of accommodation to reduce the need for hospital readmission or entering long term care
  • Promotes and engages people in positive risk taking whereby personal goals and aspirations can be achieved through outcome based planning
