HIV & AIDS SERVICE NORTH EAST LINCS
The service will support a maximum 6 individuals with a high level housing
related support service throughout the North East Lincolnshire area. The support will be delivered for a maximum of 12 months in the first instance; exception is through prior written exchange and agreement with North East Lincolnshire PCT and Supporting People Team.
The tasks to be delivered by the provider will (with the expressed fully informed written consent wish of the client and in line with agreed working protocols/information sharing). To assist clients in obtaining and maintaining suitable housing, encouraging and enabling their independent living skills;
- Prevent social exclusion through the provision of a range of support services which will enable clients to develop life skills to live independently.
- To work in conjunction with other professionals in providing clients with multi-discipline support package (i.e. Genito-Urinary Medicine Clinic, Clinical Nurse Specialists, General Practitioners, and Community Psychiatric Nurses);
- To refer and signpost clients to a range of statutory and voluntary services;
- To advocate on a clients behalf;
- To inform and advise service clients on sickness and housing benefits and support clients in identifying any other welfare benefits or grants to which they may be entitled and assist them in the application process; and.
- To visit clients at their place of residency to provide housing related support.
