Hospital Avoidance and Home from Hospital Service
North Lincolnshire

What is it?

Humbercare’s Home from Hospital Service delivers housing related support for up to 6 weeks, which is tailored to meet individuals’ needs.  The service is completely free of charge and is available 365 days per year, 7 days per week.

Who is eligible?

The service is available to all individuals aged 18 or over who:

  • Are leaving hospital.
  • Require support to avoid admittance into hospital.
  • Reside in the North Lincolnshire area.

 How Can It Help?

Humbercare will visit you on the hospital ward or at home to discuss your needs and to explain how we can support you to be safe and secure at home.  We will work with the hospital and other services to ensure you have the support you need.

  • We can support you initially with shopping and collection of prescriptions upon discharge. We can support you with initial doctor and hospital appointments.
  • We can support you with benefits advice.
  • We can assist you with sorting accumulated mail.
  • We can provide you with a temporary key safe.
  • We can signpost you to other relevant agencies for longer term support.
  • We can signpost you to community activities.
  • We can provide you with a trained volunteer befriender for up to 6 weeks until longer term  support is in place.
  • All our staff and volunteers have been checked by the Disclosure and Barring Service and they all carry  identification.Referrals

Referrals will be accepted through Hospital Discharge Teams, North Lincolnshire Council Adult Services, The First Stop Service and Substance Misuse Agencies.  Self-referrals are also welcome. Referrals can be made by telephone or email using our Home from Hospital Service referral form.
